Accident: Near Fairbanks, AK. 05-Dec-1968. Pilot undershot and landed on tundra short of frozen lake. Possibly overloaded. Substantial damage with undercarriage torn off. Pilot uninjured. NTSB report ANC69D0100.

Accident: Barrow, AK. 17-Jan-1978. Controlled collision with ground in fog. Pilot uninjured, misjudged altitude and clearance. NTSB report ANC78DA011.

Service Difficulty Report: 10-Aug-1979. A stop nut assembly found broken in lower section of control column.

N1454T Seair Alaska Airlines, Anchorage, AK. Circa 1985/1986.

Incident. Location unknown 22-May-1985. Torque dropped to 30 pounds during climb. Engine shutdown.

Accident. Location unknown. 04-Jun-1985.: Damaged floats during attempted landing on rough water. Aborted and landed elsewhere on wheels without incident

Accident: Ketchikan, AK. 25-Jun-1987. Whilst adjusting landing runway at Ketchikan Airport, pilot of the amphibious aircraft reduced power to idle and then opened up again but engine failed to respond. Pilot could not reach runway and elected to make water landing with the wheels deployed. On landing the aircraft flipped over. Substantial damage to aircraft. Pilot uninjured. NTSB report SEA87LA123.
