Accident: North of Tofino British Columbia. 49°16’N 125°58’W14-Sep-1966, The float equipped aircraft struck side of Catface Mountain at 1400ft, 7ml NE of Tofino, British Columbia. The pilot had continued VFR flight into unfavourable weather conditions. The pilot attempted to climb through a break in the clouds to clear weather above. During the climb the break closed in and the pilot then attempted to turn around and let down out of the cloud. During the descending turn the aircraft collided with trees. Pilot seriously injured and aircraft substantially damaged.

Service Difficulty Report 20-Jul-2011. On engine shutdown a sucking sound was heard as engine died. Cylinder compression check was 0/80. After staking the valve, the compression was still 0/80. Air was heard coming from the exhaust pipe. After removing the exhaust stack from the cylinder, a crack around the valve stem was noticed. Cylinder was removed from the aircraft and a serviceable cylinder was installed. Valve was removed from cylinder and a crack was seen around about 2/3 of the valve face. The valve remained intact.
