Accident: Lower Foster Lake, SK. 29-Aug-1978. The passenger deplaned during beaching and moved forward and to the right to assist in positioning the aircraft. In spite of warnings he misjudged the size of the propeller arc and was struck by the still rotating propeller.

Accident: Otter Lake, SK. 08-Oct-1979 Crashed nose down into trees after takeoff. Six occupants received fatal injuries. The overloaded aircraft was observed very shortly after lift-off to enter a left hand turn, roll to the right and quickly to the left again. It then pitched down into trees at the lakeside. The aircraft was carrying an external load consisting of rolled linoleum and possibly, but not confirmed, two folding chairs. There were only two seat belts for the six persons on board. The overload was computed to be 455lbs. It was not possible to ascertain the centre of gravity position. The accident resulted in all six persons on board receiving fatal injuries.
