Certificate of Airworthiness # 3101 & Certificate of Registration # 8031 issued 01-Jun-1949.

• CF-OCQ Province of Ontario Government, Department of Lands and Forests, Sault Ste. Marie, ON. Delivered 27-May-1949 and operated until 1964.

Accident: Chapleau River ON. 09-Dec-1949 Pilot landed on ice on the river. As he taxied in to river bank the aircraft broke through the ice suffering substantial damage.

Note: Total hours flown in 1954 -1,909.10hr.

Accident: Early 1958. An Indian passenger disobeyed the pilot’s instructions and was struck on his left arm by the propeller that had not quite come to a full stop.

• CF-OCQ Alert Bay Air Service, Alert Bay, BC. 1967-1978.

Accident: Jackson Bay, BC. 15-Dec-1970. While on final approach, the pilot reported that he hit a downdraft. He applied full power but the aircraft continued to sink until it hit the water, breaking two float struts. The aircraft bounced back into the air so he decided to fly it back to his base where he made a good landing, but the remaining strut collapsed. Substantial damage caused to the aircraft.
