CF-JMQ HOWCD Air Ltd., Edmonton, AB. Delivered 18-Jan-1957. Operated until Circa 1964.

CF-JMQ Theriault Air Services Ltd., Chapleau, ON. Based Foleyet, ON. Regd 16-Jan-1964. Deleted as retired 11-Sep-1989.

Accident: Near Beattie Lake, 5 mile north of Foleyet ON. 48.20N, 82.21W 04-Sep-1976. While in cruise flight at low altitude, the engine began to run rough, then stopped and the aircraft descended into trees. Technical inspection revealed that the spark plugs and exhaust system were coated with carbon indicating that the engine had been operating at an extremely rich mixture prior to failure. This is consistent with carburettor icing. It was also noted that there were no seats or baggage restraints for the two passengers and cargo in the main cabin. In consequence the pilot George Theriault and three passengers suffered serious injury and a fourth passenger suffered minor injury.

Accident: Lake Hood, Anchorage, AK. 26-Mar-2013. The pilot of the wheel / ski equipped airplane was taxiing for departure on a snow-covered taxiway, when he lost directional control in the deep snow, and veered to the left colliding with a parked airplane. The airplane sustained substantial damage to the left wing and lift strut. The pilot stated that there were no pre-accident mechanical anomalies that would have precluded normal operation. In the recommendation section of the NTSB Accident/Incident Reporting Form 6120.1, the operator stated that the accident may have been prevented if they had waited for the taxiway to be cleared of snow.
