Accident: Torbay Airport. NL. 18-Feb-1957. Aircraft landed normally and after slowing down a tail shimmy was apparent. The aircraft and started to taxi again but after a few hundred the shimmying started again. A visual inspection from the cockpit showed nothing amiss. The aircraft taxied to the ramp and just prior to stopping the pilot felt a drag and stopped. It was found that the tail wheel had collapsed. It seems that the tail wheel mount had sustained damage during a previous heavy landing. Minor damage with a canted bulkhead buckled.

Accident: Lac Adonis, St. Michel-des-Saints, QC. 13-Jan-2000 The ski equipped aircraft, registration C-FIVA, serial number 515, with the pilot and five French passengers on board, took off from the frozen surface of Lake Adonis, Quebec, on a pleasure flight under visual flight rules (VFR). The route had not been determined, but the flight was to last about 20 minutes. When the aircraft did not return, the search and rescue (SAR) service was advised. The aircraft was found crashed on a mountainside in a wooded area a little less than five kilometres from its point of departure. The pilot and two passengers suffered fatal injuries. The other three passengers suffered serious injuries and hypothermia. The aircraft was reported as destroyed by the force of the impact but did not catch fire. Report Number A00Q0006.
