Accident: 100 miles north of Gagnon QC, 26-Jun-1975 (Translated from French written report). The right wing had failed about eight feet from the tip  during take off and was hanging causing some violent control manoeuvres before righting itself. Obvious substantial damage to the aircraft but the pilot was uninjured.

Jul-1975 Replacement left and right wings acquired in North Carolina were fitted.

Accident: Havre St Pierre, QC. 31-Jul-1976, Crashed. The aircraft was doing a simulation of a Medevac when it encountered bad weather. The flight continued into reduced visibilty conditions, and the aircraft eventually crashed into a hill at Lat 50.19N Lon 63.02W. Pilot, Gilles Morin survived.
