Accident: Mt. Emily, Oregon. 21-Dec-1972. Suffered category 2 damage (minor/medium) during a take off roll whilst attached to the 87th Air Base Squadron, attached to the 4702nd Air Defence Wing at Geiger Field, Spokane Washington. Pilot James W. Atwood.

Accident: At Port Alexander, Alaska 18-May-1977. Amphibian gear remained down after a flight from Sitka caused by damaged hydraulic seals. Aircraft seriously damaged. Pilot uninjured. ANC77DA044
